Summary

The Program of All-Inclusive Care for the Elderly (PACE) is a community-based program that provides coordinated medical and social services to eligible older adults who want to continue living in their own home despite chronic care needs.

The Clinical Leader, RN, under direct oversight from the Medical Director, is an integral member of the management team and the PACE at Hudson Headwaters’ (PHH) Interdisciplinary Team (IDT).

The Clinical Leader oversees the delivery of nursing and other clinical services provided to Participants in the Day Center, at home, and collaborates closely with Health Center staff for care delivered in the primary care clinic.

Essential Duties and Responsibilities :

  • Direct management and oversight of clinical staff within the Day Center, home health operations both in-house and in collaboration with outside contracted partners, dieticians, social workers, and Participant Care Associates.
  • Work alongside the Medical Director to provide oversight of staff scheduling, documentation, quality of care, and collaboration with nurses (RN / LPN) and medical assistants in the Clinic.
  • Responsible for developing and maintaining clinical orientation, competency assessment, training, development, and performance appraisals for staff.
  • Participate as a member of the IDT, help facilitate IDT meetings, and provide leadership for effective care planning and problem solving for quality Participant services.
  • Ensure the Participant plan of care is executed and that all tests, procedures, consults, medications, and other necessary diagnostics are completed in a timely manner.

Ensure relevant elements of the plan of care are performed prior to subsequent clinic visits when required in the home or at the Day Center.

  • Promote and maintain collaborative relationships with managers, peers, direct reports, and Participants by effectively fostering a team environment, building consensus, and resolving conflicts.
  • Responsible for completing the initial Participant assessments upon enrollment, change of status and every 6 months. This includes assessments such as the UAS-NY (Uniform Assessment system) and PRI (Patient Review Instrument).
  • Develop and implement plans, including education and required competencies, that ensure staffing for safe high quality Participant care for all nursing, home care and personal care staff.
  • Develop and standardize clinical policies and procedures within the PACE program and in nursing practice in the community to meet standards of practice and quality assurance to successfully operate PHH.
  • Assist the PACE Management team develop and implement systems to ensure the PHH is survey ready for nursing and clinical services using action plan(s) which include(s) audit tools, rounds, and documentation review.
  • Collaborate with Medical Director to oversee the on-call system that provides 24 / 7 coverage 365 days a year to Participants.
  • Help develop and maintain department budget and place orders for equipment and supplies as necessary. Identify financial vulnerabilities and make cost reduction recommendations as needed.
  • Evaluate, develop, mentor, coach, counsel, and discipline department staff. Support personnel from other departments, as needed, through coaching and mentoring to help achieve optimum standard of excellence.
  • Participate in evaluating and integrating health care technology and information systems that support Participant care including the consistent and uniform application of the electronic health records system, care management platform and any other technology system to help meet the needs of the program.
  • Actively serve on the Quality Improvement (QI) Committee and other committees as needed. This position is directly involved with the development and implementation of committee activities.
  • Ensure that work areas are maintained in a clean, sanitary, and safe manner to meet NYS / Federal regulations and PHH policy and procedures.
  • Communicate and collaborate on a regular basis with nursing leaders at Hudson Headwaters Health Network and PHH nursing colleagues, PHH leadership, the medical staff, and other identified stakeholders for concerns related to Participant safety, quality, colleague engagement and provider and staff satisfaction.
  • Ensure compliance with healthcare standards and requirements established by various licensing, regulatory, and accreditation bodies, including Hudson Headwaters Health Network.
  • Collaborate with the PHH Program Director, VP of Finance & Contracts, Director of Quality & Compliance, Medical Director and Director of Operations & Team Services on issues related to utilization of services.

Assist with reviewing data and development of solutions to resolve any issues or concerns.

  • Ensure that all clinical areas are stocked with necessary supplies and materials and all equipment is in working order and checked according to regulations.
  • Provide overall coordination with outside contracted service providers, including hospitals, nursing facilities, assisted living facilities, home care, lab, DME, oxygen, etc.
  • Oversight of specialty clinical services which include but are not limited to dentistry, podiatry, psychiatry, audiology, and optometry.

Ensure appointments are scheduled, monitored and services are timely and comprehensive.

  • Oversee integration of new Participants into PHH, including initial assessment, medication, immunizations, routine monitoring of chronic problems, and nursing care plan development.
  • Responsible for adherence to all regulations, both federal and state, which govern the operations of a PACE organization.
  • Review Participant medical records to ensure timely and accurate clinic staff documentation is readily available.
  • Responsible for the management of infection surveillance, prevention, and control in conjunction with the Director of Quality & Compliance and Medical Director.
  • Create a work environment conducive to safe work practices, effective communication, staff collaboration, and positive morale.
  • Initiate program changes in response to changing environment and strategic direction of the program.
  • Coordinate and / or attend appropriate conferences supporting assigned program, clinic, or service.
  • Provide educational in-services to staff as necessary.

Qualifications :

The requirements listed below are representative of the knowledge, skill, and ability to perform the essential functions :

  • Bachelor’s RN degree is required
  • Current New York State RN License
  • Current BLS certification or willing to complete certification
  • 5+ years of geriatric nursing expertise preferred
  • 5+ years of supervisory health care experience, preferably in clinic with Home Care knowledge
  • Must possess a valid driver’s license
  • Ability to work within an interdisciplinary team with initiative, imagination, resourcefulness, and flexibility
  • Excellent interpersonal, verbal, and written communication skills
  • Excellent time-management skills and ability to prioritize deadlines, delegate, and make decisions
  • Demonstrated ability to work collaboratively and to maintain working relationships and team building within the PACE program and with outside partners
  • Knowledge and understanding of a team-based care approach to foster the implementation of IDT
  • Must thrive in team environment and possess excellent organizational and management skills and be able to effectively handle difficult interpersonal situations
  • Demonstrated ability to carry projects through to completion in a timely manner
  • Strong ability to manage workgroups and committees for productive outcomes
  • Proficient computer competencies including Microsoft applications, including the ability to navigate Electronic Health Record platforms;

Physical Requirements :

While performing the duties of this job, the employee is regularly required to :

  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards
  • Requires the exertion of 50 pounds of force occasionally, up to 20 pounds of force frequently, and up to 10 pounds of force constantly to move objects
  • Will require 20% sitting, 30% walking, and 50% standing
  • May also need to move heavy equipment, transfer / lift patients and assist patient with walking and turning
  • Clinical staff may also have to stoop, kneel, crouch, lift, and stand for long periods
  • May be required to travel to home locations / program and special functions as needed

The pay range for this position is $110,000 - $140,000 annually and will be determined based on skills and experience.
