Job Title : Social Worker - Home Care & Hospice

Job Summary :

We are seeking a compassionate and dedicated Social Worker to provide psychosocial assessments, care planning, and resource coordination for individuals and families. This role supports clients through a strengths-based, family-centered approach, with a focus on self-sufficiency and community integration.

Key Responsibilities :

Perform psychosocial evaluations and develop individualized care plans

Link clients to appropriate community services and support networks

Collaborate with interdisciplinary teams to coordinate care

Monitor patient conditions and communicate changes to healthcare providers

Maintain accurate documentation in compliance with agency and regulatory standards

Participate in community outreach, staff mentoring, and quality improvement initiatives

Qualifications :

Bachelor’s degree in Social Work (BSW) required; Master’s preferred

Minimum 1 year of experience in public health or a related field

Strong knowledge of community resources and service access

Effective communication and documentation skills

Ability to work independently and as part of a multidisciplinary team

CPR certification and Mandatory Reporter training (or within 30 days of hire)

Valid driver’s license, insurance, and reliable transportation required

Must pass physical exam, drug screening, TB test, and background check

Working Conditions :

Medium physical activity (up to 50 lbs occasionally)

Exposure to environmental and health-related hazards

Flexibility in scheduling and regular travel between locations required
