JOB TITLE : HARP Care Coordinator
PROGRAM : Care Coordination
STATUS : Full Time, (35 hours per week), Hybrid Position
FLSA DESIGNATION : Non-exempt
REPORTS TO : Supervisor-Care Coordination Services
DEGREE : Bachelor’s in Social Work (or related field) required
SALARY : $23.43 / Hourly Rate
EXPERIENCE : Minimum of 2 years’ experience with SPMI or mentally disabled population
- Includes On-Call Responsibilities
POSITION OVERVIEW :
The HARP Care Coordinator is responsible for supporting the Care Coordination team in all aspects of program facilitation in addition to demonstrating knowledge of the mission and
services of Pinnacle Community Services, (PCS).

Bachelor’s Social Work (or related field) requiredTwo years specialized training and experience with SPMI or mentally disabled populationExperience with linkage and coordination of community and emergency servicesExperience in substance abuse treatment valuedValid NYS Driver License and use of personal vehicle during work hoursAbility to use Microsoft applications (i.e. Word, Excel, PowerPoint, Outlook, etc.), Internet / Intranet, web-based databases. Understanding of the use of email in the workplaceJOB RESPONSIBILITIES :
Provides direct care coordination contacts to a minimum of 30 HARP eligible and / or enrolled consumersMaintains consistent and on-going contact with consumers and families while fulfilling unit of service expectations and working on their treatment plan goals with clientsConducts initial intake interview and needs assessment to determine treatment goals for Care Coordination ServicesPrepares professionally accurate and updated case documentation in accordance with regulatory, Agency, Department of Health (DOH) and Health Home requirementsPrepares statistical and administrative reports on a timely basisProvides regular, supportive client contact; emotional support; daily living counseling; legal advocacy; financial counseling; representation regarding rights and entitlements; education medical check-ups and prevention practicesAssist the Care Program with obtaining and maintaining required outcomesFacilitates linkage to appropriate services; coordination of hospitalization admission and discharge; mental health education to clients, families, and communityAssures client linkages to services for 24-hour coverageEstablishes working relationships with referral sources and other related service agenciesReviews current practices to advocate for change when needed, particularly those needed by families and minority populationsTeams with billing staff to ensure client active Medicaid status and accurate, complete client billingMeets with Program Supervisor on a regular, planned basisSUPERVISORY RESPONSIBILITIES :
