• Provide skilled nursing care under the supervision of an RN and in accordance with care plans\n
• Administer medications, treatments, and wound care as prescribed\n
• Monitor and document patients' conditions, reporting changes promptly to the RN or supervisor\n
• Assist with IV therapy, catheter care, and other skilled nursing tasks as permitted by scope\n
• Educate clients and families on medication adherence and home safety practices\n
• Maintain accurate, timely, and compliant documentation in EHR systems\n
• Promote a safe, respectful, and supportive environment for clients and families\n\nQualifications\n
• Active LPN License in the State of New Jersey (required)\n
• 1–2 years of clinical experience preferred (home care, long-term care, or related field)\n
• Excellent communication, assessment, and time-management skills\n
• Compassionate, dependable, and able to work independently in a home care setting\n
• CPR / BLS certification (required)\n
• Valid driver's license and reliable transportation\n
• Experience with Electronic Health Records (EHR) preferred\n\n??
