Your Responsibilities as a Home Health Nurse :

Deliver home-based care to patients recovering from illnesses, injuries, and chronic diseases under RN supervision

Collaborate with a dedicated team of physicians, RNs, CNAs, and aides to ensure quality patient care

Monitor patient vitals, conduct assessments, document progress, and communicate changes to your RN supervisor

Assist RNs in implementing patient care plans and teaching self-care techniques

Educate families on care plans, medications, and home safety

Work with an interdisciplinary team to coordinate patient care and discharge planning

Must-Haves for Home Health Nurses :

Active, unrestricted LPN license in the State of Michigan

At least one (1) year of clinical experience as an LPN

Current CPR Certification

Reliable transportation, valid state driver's license, and current car insurance
