POSITION SUMMARY :
The Clinical Social Worker plays a critical role in supporting individuals and families coping with grief, medical diagnoses, caregiving and life transitions. Under the supervision of the Director, the Social Worker is responsible for organizing and facilitating professionally led support groups, workshops, and emotional support sessions for teens and adults. The ideal candidate will bring experience, empathy, and initiative to help community members navigate difficult experiences such as illness, caregiving, and bereavement. This is a dynamic role requiring a compassionate, organized, and self-motivated professional who is committed to providing emotional support, resources, and education to those facing life's most challenging moments.
ESSENTIAL DUTIES & RESPONSIBILITIES :
- Plan, coordinate, and facilitate ongoing support groups for individuals dealing with illness, grief, or caregiving challenges.
- Design and lead healing-focused workshops centered around topics such as coping with loss, caregiver stress, and emotional wellness.
- Provide individual emotional support and short-term case management, including resource navigation and referrals to appropriate services.
- Collaborate with community partners, healthcare providers, and internal departments to ensure holistic support for participants.
- Maintain accurate documentation and group records in accordance with program protocols.
- Stay current on community resources, emerging best practices in grief and illness support, and identify appropriate materials and tools to assist clients.
- Attend regular supervision and team meetings.
- Perform other related duties as assigned by the supervisor.
KNOWLEDGE, SKILLS, & ABILITIES :
Must-Have
Proven experience facilitating support groups, especially for individuals facing medical issues, caregiving, grief, or bereavement.Strong knowledge of local and national support resources for medical, grief, and wellness services.Excellent interpersonal and communication skills with a compassionate and professional demeanor.Flexibility in schedulingEDUCATION & EXPERIENCE :
Minimum of 5-7 years of experience working in a healthcare, hospice, wellness, or similar community-based setting.Master's degree in Social Work (MSW) required; valid LMSW or LCSW in New York State.This is an approximate of the shift : Total Weekly Hours : 20-25 hours
Evening Hours :2-3 evenings per weekEach evening : 1.5-2.5 hoursTotal evening hours per week : approx. 3-7.5 hoursDaytime Hours :The remaining hours are during the dayTotal daytime hours per week : approx. 12.5-22 hoursCompensation details : 30-40 Hourly Wage
