Unit Details :

Patient Ratios Days : 20

Patient Age Groups : Adolescents, Adults, Pediatrics, Geriatrics

Daily Census : case load up to 25

Interdisciplinary Support : Social Services

Patient Diagnoses : HARDSTOP- ACUTE CARE Hospital Case Management EXPERIENCE AND MUST BE CURRENT WITHIN THE LAST YEAR.

Special Procedures / Unit Details : REQUIRED : Experience with HMOs, IPAs, etc.

Experience with advance directive, POLST, protective services process.
