As a Medical Social Worker at CenterWell Home Health, you will play a vital role in providing interdisciplinary care to home health patients. Your expertise will help evaluate and develop personalized care plans that address the emotional and social needs of our patients.

Your Responsibilities

In this rewarding position, you will :

Assess patients’ social and emotional states related to their illness or injury, and their responses to treatment.

Evaluate the interplay between medical needs, financial resources, and community support.

Identify and connect patients with community resources to facilitate their recovery.

Guide patients and families in coping with social and emotional challenges.

Collaborate with physicians and health team members to address significant social factors affecting health.

Monitor and report changes in patients’ conditions to the Clinical Manager.

Participate in discharge planning and care coordination activities.

Supervise and evaluate the performance of the Social Work Assistant.

Maintain accurate documentation as required by company policies.

What You Bring to the Table

We are looking for candidates who possess :

A Master’s or Doctoral degree in Social Work from an accredited institution.

Licensure as a Social Worker in your state, as required by law.

A valid driver’s license, auto insurance, and reliable transportation.

Current CPR certification.

A minimum of one year of experience in a healthcare setting, preferably in home health or hospice.

Knowledge of discharge planning and community resources.

Excellent communication and interpersonal skills.

Work Schedule and Compensation

Scheduled Weekly Hours : 20

Pay Range : $59,300 - $80,900 per year

This range reflects a good faith estimate for full-time employment and may vary based on location and individual qualifications.
