Job Purpose

The Transportation Coordinator plays a key role in ensuring safe, reliable, and timely transportation for PACE participants to and from the day center, medical appointments, and other essential destinations. This role involves scheduling, routing, dispatching, and overseeing transportation staff and vendors while maintaining compliance with safety regulations and PACE standards of care. The Coordinator supports the overall well-being and independence of older adults by managing the transportation logistics that enable them to access critical services.

Duties and Responsibilities

  • Coordinate daily transportation services for PACE participants, including center visits, medical appointments, and social activities.
  • Develop and optimize driver schedules and vehicle routes to maximize efficiency and minimize delays.
  • Supervise and support drivers and transportation aides; provide training and performance feedback.
  • Communicate regularly with interdisciplinary team members (IDT) to ensure transportation aligns with participants’ care plans.
  • Serve as primary point of contact for all transportation-related issues and respond to changes in schedule, weather, or participant needs.
  • Maintain accurate records of trips, mileage, and participant transportation logs in accordance with PACE program requirements.
  • Ensure all vehicles meet safety, maintenance, and regulatory compliance standards.
  • Coordinate vehicle inspections, repairs, and preventative maintenance with internal staff or external vendors.
  • Collaborate with third-party transportation providers as needed, ensuring adherence to service quality and participant safety.
  • Monitor transportation-related incidents and support investigations and resolutions as necessary.
  • Participate in emergency preparedness planning related to transportation services.
  • Promote a culture of respect, safety, and dignity for all PACE participants during transport.
  • Provide backup driving services in the event of driver absence due to illness, vacation, or staff shortages to ensure uninterrupted participant transportation.
  • Other duties as assigned.

Qualifications

  • High School Diploma or equivalency preferred.
  • Excellent organizational, communication, and problem-solving skills.
  • Strong working knowledge of routing software, GPS system, and Microsoft Office Suite.
  • Ability to work collaboratively with interdisciplinary teams and adapt to changing participant needs.
  • Valid driver’s license with clean driving record, CDL preferred by not required.
  • Ability to pass background check and health screenings in accordance with state and federal regulations.
  • Experience :

  • Must have Driver’s License for at least five (5) years with no more than one (1) moving violation or one (1) at-fault accident in the last three (3) years
  • Experience in non-emergency transportation or public transportation preferred
  • Must be comfortable using technology
  • Knowledgeable of PACE program principles and regulatory guidelines (highly preferred).
  • Skills and Knowledge :

  • Effective written and oral communication skills.
  • Demonstrated ability to problem solve and apply good judgment.
  • Ability to function well independently.
  • Ability to use or be trained in computer skills for the workplace.
  • Demonstrates necessary skills and knowledge as outlined in attached position-specific Competency Assessment Profile.
  • Able to speak Cantonese / Mandarin / Spanish preferred.
  • Other :

  • Requires physical strength to perform the job.
  • Routine travel between PACE sites and in the community required.
  • Requires valid driver’s license.
  • Physical Demands

  • Must be able to remain in a stationary position 50% of the time.
  • Ability to occasionally move about inside the office to access file cabinets, office machinery, etc.
  • Able to operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.
  • Able to constantly position yourself to maintain files in file cabinets such as reaching with hands and arms, kneeling, crouching, etc.
  • The ability to communicate, detect, converse with, discern, convey, express oneself, and exchange information is crucial for this role.
