The key to success in this role lies in your ability to :

Collaborate with HCBS Supervisors and Programmers to develop personalized goals for the individuals served.

Provide tailored services based on each individual's unique goals and behavioral care plan, which includes :

o Teaching individuals how to accomplish their objectives rather than completing tasks for them.

o Utilizing behavioral strategies that support individuals and address any behaviors hindering their success.

o Motivating and encouraging individuals to participate in their behavioral care plans and strive for excellence.

o Participating in recreational, community, and social activities with those you serve, respecting the choices of the individual.

o Demonstrating an understanding of when it is appropriate to apply rights-restrictions, as determined by the interdisciplinary team and indicated in the individual's behavioral care plan.

Providing thorough, consistent, and accurate documentation of the individual's progress.

Creating safe environments and instructing individuals served on practices that will aid them in reaching their goals.

Offering care that enables individuals to present themselves confidently to the community, which may include assistance with hygienic needs, choosing suitable clothing, administering medication, and ensuring access to food options catering to their dietary requirements.

Acting as an advocate for individuals receiving services, promoting their autonomy and empowerment to live their lives as independently as possible.
