San Luis Valley Health has an opening for an experienced Advanced Practice Provider in our La Jara Clinic. A successful candidate must be a Board Certified or Board Eligible Nurse Practitioner or Physician Assistant and possess a desire and ability to work with a team in a collegial setting to provide comprehensive medical care.
Minimum Requirements :
- Current state license as a Nurse Practitioner or Physician Assistant (credentialing requirement).
- Eligible for State Controlled Substance License
- Eligible for Federal DEA
- BLS Certification
