Delaware Hospice is actively seeking a full-time Admissions RN to cover New Castle County.\n\nWe invite you to explore a Delaware Hospice career to rediscover the hands-on, patient centered care that drew you to the nursing profession in the first place. Our Admissions Nurses are an integral part of Delaware Hospice's mission and interdisciplinary team.\n\nQUALIFICATIONS : Current license to practice professional nursing in the State of Delaware (in New Castle County, must maintain PA license as well). Graduate of an approved school of nursing. Bachelor's degree in nursing preferred. Certification in hospice nursing preferred. Minimum of two years of nursing, preferably in hospice or home care.\n\nRESPONSIBILITIES : Functions as a member of the interdisciplinary team to provide Hospice services to patients and families. Acts as a representative of Delaware Hospice in the hospitals and facilities we serve. Establishes and promotes an effective positive relationship with hospital and facility staff. Manages General Inpatient visits in the hospitals. Utilizes the nursing process to provide quality nursing care to Hospice patients in accordance with Delaware Hospice policy. Instrumental in facilitating efficient admission of patient and families to Delaware Hospice.\n

• Delaware Hospice nurses may care for pediatric patients.\n
• We offer specialized training and orientation for nurses who wish to be part of the Katybug team.\n
• Our Katybug program specializes in helping kids from perinatal to young adult, find relief from pain and symptoms in their own home, while offering families the practical resources and emotional support they need to cope.\n\nYou will be part of an entire team of highly qualified, caring individuals assists with the physical, emotional, and spiritual needs of you and your loved ones. The dedicated Delaware Hospice team includes : \n
• Medical Director : Consults with the primary physician, oversees the care plan, and, if necessary, may serve as the attending physician.\n
• Registered Nurse : Acts as the primary point of contact, conducts weekly medical assessments, manages the patient's care plan, lets the family / caregiver know what to expect, and answers all questions.\n
• Certified Nursing Assistant : Provides personal care to the patient and keeps the family / caregiver informed and advised.\n
• Social Worker : Provides emotional care and supports the needs of the whole family.\n
• Chaplain : Offers spiritual direction and prayer.\n
• Adult and Child Bereavement Counselor : Provides grief support.\n
