Counselor - IP
Position Summary :
This position of Counselor / Clinical Case Manager will provide supportive counseling and education to help support patients and families who are experiencing emotional, financial, and psycho-social concerns following spinal cord, acquired brain injury, and other neurological disorders. Clinical Care Managers are : patient advocates, team leaders / liaisons, clinical team members, counselors, liaisons with insurance and other funding resources, and discharge planners.

Rate of Pay : $33.50 - $48.23 per hour based on experience
Position Requirements :
- Licensure / Certification / Registrations : None.
- Education : Master's Degree from an accredited college or university in Social Work, Licensed Professional Counselor, Rehabilitation Counseling or in a related field. Specialized training in rehabilitation or equivalent experience working with people who are spinal cord injured and acquired brain injury.
- Experience : 3–5 years of experience in a rehabilitation or hospital setting with a neurologic population preferred.
- Skills & Abilities : Ability to successfully handle multiple priorities and schedule accordingly while meeting established deadlines. Must be able to successfully and effectively communicate with a variety of personnel. Problem solve complex medical , legal, and discharge issues. Effectively manage conflict between family members. Flexible in changing discharge planning due to insurance denials. Able to maintain a high degree of accuracy, dependability, and strong attention to detail both mechanically and technically. Ability to maintain confidentiality in all aspects of the position. Computer skills — Epic, Microsoft Office.
Essential Functions :
Assist and educate patients / families in accessing financial resources.Individual and family counselingIdentifies community resources to help patients discharge from rehabilitation and resume community living.Recognize patient and family goals and provides pertinent education through individual and group counseling.Communicate with hospital personnel both verbally and in writing about significant psychosocial factors that may affect the patient's progress.Writes reports and letters and maintains pertinent records of services provided.Participate in departmental and interdepartmental planning to improve medical / rehabilitative services.Perform in accordance with hospital and departmental policy and procedures.Provide services to adolescent, adult and geriatric patients and adapt the service as needed.Cultural awareness to meet all patient’s needs.This job description is not intended to be an exhaustive list of all duties.
