job_description.job_card.job_descriptionA : High exposure to blood borne pathogens Graduate of an approved professional nursing program (RN). Currently licensed in good standing in Pennsylvania. Minimum of one (1) year of nursing experience, preferably in home care. Receive intake referral information and prepare documentation for visits. Meet with potential clients to assess care needs, develop a personalized plan of care, and conduct home safety checks. Conduct 30-day follow-up calls and 60-day in-person visits for reassessment. Adjust care plans as needed based on reassessments and client / family feedback. Communicate effectively with clients, families, staff, and referral sources. Develop, implement, and revise individualized Plans of Care (POC). Supervise DCWs in compliance with PA DOH / DHS regulations. Order and adjust professional services as needed. Educate clients / families about billing and service coverage. Communicate effectively with clients, families, physicians, and interdisciplinary teams. Submit accurate weekly paperwork and complete assessments within 48 hours. Participate in clinical record reviews, staff meetings, and QA processes. Precept new hires and contribute to growth of the office. Maintain professional licensure and ongoing education. Report emergencies / sickness promptly and submit time-off requests in advance. Ability to lift / transfer clients up to 150+ pounds. Able to carry bundles up to 10 pounds upstairs. Must stoop, bend, and travel for client visits. Must hear and effectively communicate in English. Active Pennsylvania RN license. Proficiency with computer and office equipment.
Job Summary : A nurse licensed and registered in Pennsylvania who provides professional services within nursing practice standards, collaborates with physicians, referral sources, and educates clients / families.
