About The Position

Job Title : Hospice RN

Hours per Shift : 8 : 00 AM - 4 : 30 PM, occasional weekends

Experience : Minimum 1 year of nursing experience required.

License : Current unencumbered Oregon RN Licensure required.

Certifications : Healthcare Provider Level BLS required.

Must-Have : BSN and / or previous home health / hospice or case management experience preferred.

Description :

The Hospice teams support patients and family members through the final stages of illness.

Services are offered in the home setting, which may include the patient's residence, skilled nursing facilities, assisted living facilities, and adult foster homes.

Services provided include intermittent skilled nursing visits, with 24-hour on-call nursing services available.

Pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, and bereavement support are also included.

Medications and equipment are provided as appropriate.

This position involves working between multiple locations, depending on patient needs.

This position requires the use of the employee's personal automobile.

The employee must have a valid driver's license and auto liability / property damage insurance as required by law.

Maintaining the vehicle in good working order is essential.
