Summary

The Patient Care Coordinator is the point of contact to ensure patients' needs are met. The PCC coordinates patients' treatment plans and follow up, order changes, and renewals.

Essential Functions

  • Determines insurance eligibility and coverage for a variety of plans and patient-specific situations
  • Submits authorization requests to insurance in an accurate and timely manner
  • Communicates with patients and families to assess supply needs
  • Screens / rescreens patients for nutrition risk per established criteria
  • Documents data collection results and makes appropriate referrals to the Dietitian as per
  • Communicates effectively with physicians and dietitians regarding patient needs and required documentation
  • Works directly with staff, patients, insurance companies and referral sources to ensure the best possible outcomes for our patients
  • Documents communication and action plans thoroughly
  • Troubleshoots product sourcing issues if applicable
  • Special Projects as assigned
  • Process customer orders, requests and information updates into a patient order management system
  • Performs order changes and renewals
  • Preferred Education and Experience

  • Associate degree or Equivalent
  • Able to work in fast-paced, self-directed environment
  • PC literate – Ability to use Excel, Word, PowerPoint & nutrition software - Brightree experience a plus

    Work Environment & Physical Demands

    In Office - 685 Citadel Drive East, Suite #530, Colorado Springs, CO 80909. This role is in a professional office setting with moderate noise level. The position requires regular use of standard office equipment such as computers, phones, photocopiers, filling cabinets, and fax machines. The work environment is fast-paced and may involve frequent interruptions, shifting priorities, and regular interaction with patients, families, referral sources, and clinical staff. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the role.

    The position is primarily sedentary and requires the ability to sit for extended periods of time while performing computer-based tasks and telephone communications. The role involved frequent use of hands and fingers to operate computer keyboard, mouse, and other office equipment. The employee must be able to effectively communicate verbally and in writing. Occasional standing, walking, reaching, and light lifting (up to 10 pounds) may be required.

    Position Type / Expected Hours of Work

    This is a full-time position. Days and hours of work are Monday through Friday, 8 : 00 a.m. to 5 p.m. and on-call.
