Schedule : Monday-Thursday 7 : 00am-7 : 00pm

Location / Setting : Fort Walton Beach 32547

Age Range : Child

Acuity : High, Trach / Vent, Lifting Required (mechanical lift)

Patient 2

Schedule : Saturday / Sunday 7 : 00am-11 : 00pm

Location / Setting : Fort Walton Beach 32547

Age Range : Child

Acuity : Low, Feeding Tube

Patient 3

Schedule : Monday / Tuesday 7 : 00pm-7 : 00am

Location / Setting : Fort Walton Beach 32547

Age Range : Toddler

Acuity : High, Trach / Vent, Lifting Required
