About the Opportunity

A rapidly growing mobile healthcare provider is seeking a Wound Care Nurse Practitioner (NP) to join its mission-driven team delivering high-quality, evidence-based care in the Xenia, OH. You’ll provide direct wound care and primary care services to patients in nursing facilities.

What You’ll Do

• Deliver wound care and general primary care services to adult and geriatric patients
• Conduct assessments, manage acute / chronic conditions, and create individualized treatment plans
• Perform advanced wound care procedures including debridement, dressing changes, and infection management
• Educate patients and caregivers on wound prevention and health maintenance
• Collaborate with interdisciplinary teams to ensure coordinated care
• Maintain timely and accurate documentation
• Shadow and train closely with a peer provider before practicing independently
