What You’ll Do :
As our new Occupational Therapist , you’ll be at the heart of our care team, delivering personalized therapy services that empower patients to live more comfortably and confidently. Your responsibilities will include :
- Providing direct therapy treatments in patients’ homes
- Assessing home environments and developing personalized care plans
- Collaborating with physicians and other healthcare professionals
- Monitoring and reporting patient progress and adapting plans as needed
- Identifying additional care needs and coordinating with home health services
- Supervising OT assistants in accordance with policy and regulations
- Supporting patients and families through discharge planning and beyond
