Job Description & Requirements Specialty : Med Surg Discipline : RN Start Date : ASAP Duration : 7 weeks 40 hours per week Shift : 8 hours, nights Employment Type : Local Contract Job Title : Licensed Nursing Assistant (LNA) Facility : Elm Wood Center Shift : 3rd Shift (11 : 00 PM – 7 : 00 AM) Position Summary : Under the direction and supervision of a Registered Nurse (RN), the Licensed Nursing Assistant (LNA) delivers efficient and effective nursing care to achieve positive clinical outcomes and patient / family satisfaction. The LNA operates within the scope of practice defined by the State Nurse Practice Act and supports the nursing team by providing direct patient care and assisting in maintaining a safe, comfortable, and therapeutic environment for residents. Responsibilities and Accountabilities : 1. Patient Evaluation & Observation : Collects, reports, and documents objective and subjective data. Observes changes in residents' conditions and promptly reports them to the RN. 2. Care Planning & Implementation : Contributes to establishing individualized patient goals. Assists in developing and implementing interventions to meet patient needs. Participates in evaluating care outcomes and revising care plans as needed. 3. Direct Patient Care : Provides personal care assistance including bathing, dressing, grooming, feeding, and mobility support. Takes and records vital signs, weights, and other routine measurements. Assists with medication administration and treatments under the direction of the RN / LPN. Communicates effectively with patients, families, and the healthcare team. Documents care accurately and timely in Point Click Care (PCC). 4. Supervision and Teamwork : Supports and collaborates with RNs, LPNs, and other healthcare staff to deliver high-quality care. Participates in shift-to-shift handoff communications to ensure continuity of care. Helps train and orient new staff as assigned. 5. Safety & Compliance : Follows infection control, safety, and confidentiality policies at all times. Maintains a clean, safe, and organized work environment. Reports safety hazards, incidents, and non-compliance concerns promptly. Participates in quality assurance, compliance, and in-service training programs. 6. Additional Duties : Assists physicians during rounds and examinations when needed. Works beyond scheduled shifts when required to meet staffing or patient care needs. Performs other duties as assigned by the supervisor or RN. Compliance : Adheres to all applicable state and federal regulations, including HIPAA and the Federal False Claims Act. Participates in the Compliance and Ethics Program and required training sessions. Cooperates with audits, investigations, and performance improvement initiatives. Educational / Vocational Requirements : Graduate of an approved school for practical or vocational nursing. Must hold an active State Board of Nursing License. Experience in rehabilitative or geriatric nursing preferred. Must be able to read, write, and communicate effectively in English. Must provide verification of Tuberculin Skin Test (TST) as required.
