This role is ideal for a mission-driven RN ready to transform lives and community healthcare in NYC.\n\nTitle : Registered Nurse – Community-Based Homeless Services\n\nLocation : New York City, NY (Field-based)\n\nSalary + Benefits : Competitive salary, comprehensive health coverage, generous PTO, flexible hours, and the opportunity to be part of a high-impact statewide initiative.\n\nResponsibilities of Psychiatric Registered Nurse – Community-Based Homeless Services : \n\nConduct assessments of participants' physical, cognitive, and psychosocial health needs in field settings.\n\nEngage participants in non-traditional settings (transit system, shelters, hospitals) and assist with transition planning.\n\nDevelop, implement, and evaluate care plans and interventions based on evidence-based practices.\n\nReassess participants regularly, especially when changes in condition or needs are observed.\n\nEducate participants on health maintenance, disease prevention, diet, physical activity, and medication use.\n\nLink participants to appropriate medical, specialty, and social service providers in the community.\n\nProvide physical and psychological support in-home and community settings.\n\nCollaborate closely with licensed clinicians, care managers, and peer specialists to coordinate holistic care.\n\nAdvocate for participants' health needs across acute and non-acute healthcare systems.\n\nRecord medical information, vital signs, symptoms, and changes in condition accurately and timely.\n\nParticipate in team meetings, provide nursing insights into treatment planning, and engage in risk management.\n\nPromote awareness of available community resources and work to reduce health disparities.\n\nSupport with medication reconciliation and monitoring for side effects.\n\nPerform other nursing duties as needed in the field.\n\nRequirements of Psychiatric Registered Nurse – Community-Based Homeless Services : \n\nCurrent license and registration to practice as a Registered Nurse in New York State.\n\nMinimum of 1 year of clinical nursing experience post-licensure, preferably working with individuals with behavioral health needs.\n\nStrong communication, interpersonal, and organizational skills.\n\nWillingness to work in a field-based role with flexibility in scheduling.\n\nPassion for serving underserved populations and improving community health outcomes.\n\nJob Type : Full-time\n\nPay : $112,000.00 - $115,000.00 per year\n\nBenefits : \n
