Role Description

This is a full-time on-site role for a Homecare Registered Nurse. The Homecare RN will be responsible for providing high-quality nursing and patient care in a home care setting. Day-to-day tasks include administering medication, wound care, monitoring patient health, and collaborating with other healthcare professionals to develop patient care plans.

Qualifications

• Complete an initial home assessment of patient and family to determine home care needs.
• Provide a complete physical assessment and history of current and previous illness(es).
• Utilize the nursing process in planning, providing, and evaluating skilled needs.
• Complete a comprehensive patient assessment that is reflected in accurate OASIS scoring.
• Develop a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventative, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process.
• Coordinate goal directed and cost-effective care for patients. 6. Demonstrate appropriate utilization of needed visits.
• Counsel the patient and family in meeting nursing and related needs.
