Social Services Specialist III - GALT

This position is located in Oklahoma City, Oklahoma.

Social Services Specialist III H20C Pay Rate :

• $23.25 PER HOUR   /   PART-TIME 20 HOUR PER WEEK THIS POSITION IS A PART-TIME CONTRACTOR POSITION WITH THE POTENTIAL OF PERMANENT FULL-TIME STATE EMPLOYMENT .
• Travel is OCCASIONAL.
• Must possess a valid drivers license and must maintain required car insurance.   Minimum Qualifications Education and Experience requirements at this level consist of a master’s degree plus two additional years of qualifying experience; or a bachelor’s degree and three years of experience in professional social work; or an equivalent combination of education and experience, substituting one additional year of experience in professional social work for each year of the bachelor’s degree.   Job Responsibilities Provides comprehensive HIV case management to individuals & families living with HIV, with a targeted outreach emphasis to Native American Tribal Health facilities and on at risk and under served populations throughout Oklahoma.
• Facilitates & provides access to federal & privately funded prescription assistance programs, specialized home health programs, COBRA benefits, & medical insurance; Initiate and facilitate expedited Medicaid Waiver services for clients with urgent medical needs.
• Provide comprehensive services plan development with the client in conjunction with APS, CWS, and AFS agency staff.
• Refer & schedule client appointments for medical services; Serve as gatekeeper for client access to emergency community funds and pantries.
• Collaborate with other agencies through monthly inter agency staff meetings to identify, develop, and maintain client services.
• Identify and build partnerships with AIDS Service Organizations.
