IDR Healthcare is seeking a travel Home Health Physical Therapist for a travel job in Baltimore, Maryland.

Position : Physical Therapist (PT) – Home Health

Location : Baltimore, MD

Start Date : ASAP

Contract Type : Contract-to-Hire

Work Schedule : Flexible days with unit-based productivity

Expected Units : 30 units / week

Coverage Area : Baltimore City and surrounding areas (may be reassigned nearby if census is low)

Join a compassionate home health care team serving patients across Baltimore. This role is ideal for a skilled Physical Therapist who thrives in autonomous fieldwork and enjoys helping patients reach their maximum functional potential in a home setting.

🧠 Responsibilities & Role Overview

  • Perform physical therapy evaluations and deliver treatment plans based on physician referrals
  • Conduct in-home assessments and establish measurable treatment goals
  • Modify goals and treatment plans based on patient progress
  • Collaborate with interdisciplinary care teams and physicians to coordinate care
  • Educate patients and families on therapy goals and home programs
  • Recommend and assist with adaptive equipment
  • Document evaluations, care plans, and treatment visits in EMR within required timelines
  • Adhere to all HIPAA, compliance, and state / federal regulations
  • May supervise PTAs and Aides in accordance with state law
  • Participate in internal clinical development and CE programs

    • ✅ Requirements

  • Active Physical Therapist license in the state of Maryland
  • Recent Home Health experience required
  • Familiarity with OASIS documentation required
  • Experience with adaptive equipment, post-acute care, and chronic condition management
  • Strong ability to work independently in the field
  • Valid driver’s license and reliable transportation

    • HCHB (HomeCare HomeBase) EMR experience preferred

    Candidate may be reassigned to nearby counties if census is low

    Mileage reimbursement details provided during onboarding

    Candidates must be comfortable meeting weekly productivity standards

    This is an onsite, field-based position (not remote)

    Contract-to-hire opportunity for strong performers

    Scrub colors not specified; professional attire required
