About the Role :

As a Licensed Vocational Nurse (LVN) at Arbrook Plaza Skilled Nursing, you will play a critical role in delivering compassionate and high-quality nursing care to our residents. Your primary focus will be to support the healthcare team by providing direct patient care, monitoring patient health, and assisting with medical procedures under the supervision of registered nurses and physicians. You will ensure that each resident's physical and emotional needs are met with professionalism and empathy, contributing to their overall well-being and recovery. This role requires meticulous attention to detail, strong communication skills, and the ability to work collaboratively within a multidisciplinary team. Ultimately, your efforts will help maintain a safe, comfortable, and therapeutic environment for all residents in our skilled nursing facility.

Minimum Qualifications :

  • Valid and current Licensed Vocational Nurse (LVN) license in the state of employment.
  • Completion of an accredited vocational nursing program.
  • Basic Life Support (BLS) certification.
  • Demonstrated knowledge of nursing principles, patient care techniques, and medical terminology.
  • Ability to pass background checks and meet health screening requirements.

Preferred Qualifications :

  • Experience working in a skilled nursing or long-term care facility.
  • Additional certifications such as IV therapy or wound care specialty.
  • Familiarity with electronic health record (EHR) systems.
  • Strong interpersonal skills with experience in patient and family education.
  • Continuing education or training in geriatric nursing or chronic disease management.
  • Responsibilities :

  • Provide direct nursing care to residents, including administering medications, wound care, and monitoring vital signs.
  • Assist in developing and implementing individualized care plans in coordination with registered nurses and healthcare providers.
  • Document patient information accurately and timely in medical records to ensure continuity of care.
  • Observe and report changes in residents’ conditions to the healthcare team promptly.
  • Support residents with activities of daily living such as bathing, dressing, and mobility assistance.
  • Maintain a clean and safe environment by adhering to infection control protocols and facility policies.
  • Communicate effectively with residents, families, and healthcare team members to promote understanding and comfort.
  • Skills :

    The required skills enable the LVN to perform clinical tasks such as administering medications, monitoring patient status, and documenting care accurately, which are essential for maintaining high standards of patient safety and care quality. Effective communication skills are used daily to interact compassionately with residents and their families, ensuring their concerns are heard and addressed. Critical thinking and observational skills help in identifying changes in patient conditions promptly, allowing for timely interventions. Preferred skills, such as proficiency with electronic health records, enhance efficiency in documentation and coordination with the healthcare team. Additionally, specialized knowledge in wound care or IV therapy allows the LVN to provide advanced care, improving patient outcomes and supporting the facility’s comprehensive care approach.
