Concord Care Center of Toledo responds to the needs of those residents with mental illness, mild to moderate dementia, developmental disabilities, and other behavioral needs. The facility is dedicated in design and services to caring for this specialized segment of the long term care community as well as those seeking short-term rehabilitation. The goal of our specially trained staff is to deliver care and services with patience and innovation, while promoting quality of life for all of our residents. The State Testes Nursing Assistant (STNA) working under the direction of their charge nurse assists in the delivery of care to assigned residents. The STNA is responsible for maintaining neat and clean resident care and work environments to ensure sanitary conditions. The STNA is responsible for ensuring that residents are comfortable and are satisfied with their level of service. The STNA is responsible for answering call lights, taking vital signs and should have the ability to recognize irregularities in assigned residents in order to report them to the RN or LPN other medical professional when needed. The STNA is also responsible assisting with activities of daily living including but not limited to : resident hygiene (which can include bathing / showering, hair care, perineal care to both males and females, oral care (including dentures), nail care, and shaving), and dressing and undressing residents. They assist with ambulating, turning, and repositioning as needed and indicated per their plan of care.
The STNA will also be responsible for assisting residents with serving meals, snacks, and fresh water to ensure that the resident is provided nutrition through supervision or extensive assistance based on individual resident need. We are currently looking for a full time and part time STNAs on all shifts. The STNA documents actions by completing forms, reports, logs, and records.
