Minimum Qualifications :

Current New York State Home Health Aide (HHA) certification

Job Duties :

Escort residents to and from clinic appointments and dialysis sessions

Monitor and prevent falls and altercations, intervening immediately

Assist residents in preparing for appointments and ensure they receive snacks during out-on-pass periods

Report incidents and accidents promptly to the clinical coordinator or ADNS

Act as a representative of the facility, adhering to all policies and procedures on and off the property
