In this vital role, you will collaborate with a multidisciplinary team and deliver professional nursing care directly to patients. Your responsibilities will include performing admission visits, assessing patient eligibility for home care services, and developing individualized care plans.\n\nQualifications : \n

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.\n
• Current state license as a Registered Nurse.\n
• Current state Driver's License.\n
• One year of experience as a Registered Nurse; two years preferred.\n
• Proof of current CPR certification and Hepatitis consent / declination.\n
