Job Summary

The Comprehensive Prenatal Health Worker I (CPHW I) provides support services, ongoing orientations, assessments, care plan development, appropriate nutrition, health education and psychosocial interventions and referral when needed, to UCHC’s prenatal patients.

Essential Functions

▪Conducts face-to face interviews with all prenatal patients, screens patients for presumptive eligibility (PE) and issuing of PE cardif eligible; Issues the 2nd good thru extension for patients.

▪Identify opportunities and facilitate efforts to expand service or increase patient access into programs.

▪Assures client charts are documented whenever telephone & face-to-face contact is made.

▪Codes all super bills accordingly to type of visit and signs all chart entries using legal signature and title.

▪Provides ongoing orientation, assessments, classes and makes the appropriate referrals when needed to our prenatal patients.

▪Prepares all Intake packets English / Spanish and makes sure all appointments have been given to patient accordingly. i.e.,Intake, Initial, Medi-Cal, etc.

▪Assist patients in completing Medi-Cal forms (application) and follow-up on no show / missed appointments.

▪Orders all prenatal forms as needed and fax weekly PE enrollment Summary Forms to the state.

▪Fax weekly schedules to Medi-Cal worker and advocate for patients with the Medi-Cal worker.

▪Case management of the prenatal program, which includes reviewing all prenatal charts after each clinic visit with the provider, aswell as reviewing each chart the day before the visit with the provider. This is to assure that all procedures, tests, etc. that theprovider has ordered have been done and lab results have been received on a timely manner.

▪Attends recommended job-related trainings provided by the CPSP County program.

Non-Essential Functions

▪As directed by the Associate Medical Director / Director of Women's Health, performs other related and / or necessary tasks.

Qualifications

▪High school diploma or GED required.

▪Medical Assistant (MA) certificate preferred.

▪Current Basic Life Support▪Zero (0) to two (2) years of experience in a community health center setting.

▪Certification as a community health worker preferred. ▪Bilingual in English / Spanish highly preferred.

▪Requires a comprehensive knowledge of prenatal care in outpatient settings.

▪ Maintain current knowledge of policies and procedures as they relate to safe work practices.

▪ Follow all safety procedures and report unsafe conditions.

▪ Follow all infection control procedures including blood-borne pathogen protocols

▪ Ability to work with a diverse population.

▪ Willing to work flexible hours, including evening and / or weekends.

▪ Must have reliable transportation.

▪ Must be self-directed and work productively with minimal supervision.

▪ Good communication skills are necessary.

Physical Requirements and Environmental Conditions

▪ Exposure to blood and body fluids, communicable diseases, chemicals, radiation, and repetitive motions

▪ Pushing and pulling objects up to 25 lbs.

▪ Frequent wrist, hand and finger dexterity to perform fine motor function without tremor.

▪ Full range of body motion including twisting body, pushing, and pulling.

▪ Position requires light to moderate work with 25 lb. maximum weight to lift and carry.

▪ Position requires reaching, bending, stooping, and handling objects with hands and / or fingers, talking and / or hearing, and seeing.

Working Conditions

▪ Non-Ionizing Radiation (microwaves, sun)

▪ Hazardous Exposure (chemical [E.G. Latex] and infectious)

Work Schedule

▪ You will generally be scheduled to work each day Monday through Friday, with starting times each day varying between 8 : 00 a.m. and 9 : 00 a.m. and ending time between 5 : 00 p.m. and 6 : 00 p.m., as required to meet the operational needs of the company. There will be occasional Saturdays as needed.

▪ You will need to travel to other clinic locations as needed for cross coverage support.
