Start Date of April 30th No Call or Weekends

Opportunity :

• Full time schedule, Monday - Friday, 8am-5pm
• No Call or Weekends
• Adults only population
• Multi-disciplinary care approach
• Transitional care plan development
• Treatment of acute and chronic conditions
• 12-14 patients per day
• Active PA license required
