Location : Hybrid - Palm Beach County

We're seeking experienced and compassionate Behavioral Health Pediatric Care Manager to join a respected, nationally recognized healthcare organization. Daily Responsibilities for Behavioral Health Pediatric Care Manager

  • Assess members' medical, behavioral, and social needs through phone, virtual, or in-home visits to create personalized care plans.
  • Coordinate services between members, families, healthcare providers, and community resources to ensure timely, quality care.
  • Monitor progress, update care plans as needed, and educate members on treatment options and available support programs.

  • State of Florida LCSW, LMHC, LPC
  • Must live in Florida

    • Benefits For Remote Behavioral Health Pediatric Care Manager

  • Compensation : $70,000 - $100,000 per year
  • Full-time, permanent position
  • Monday–Friday daytime schedule
  • Hybrid Schedule, strong work / life balance with predictable hours
  • Annual bonus eligibility
