The primary function of the Lifestyle Coach is to plan, organize, develop, and conduct assessments of the health and overall needs of the LifeChoices members and prospective members. Develop individual wellness plans, assist in developing individual care coordination plans for care services, assess for wellness interventions, and oversee the implementation of all the plans. Establish relationships with providers and oversee the services delivered to members. Provide education and assistance to members as needed and assist with all other program operations including administrative needs. This position reports to the Executive Director of LifeChoices, EHM Senior Solutions .
Educational & Experience Requirements
o A high school education is required. Bachelor’s degree in Social Work, Psychology, Gerontology, Human Services, Nursing, or Occupational Therapy preferred.
o Must have prior experience working with older adults in the home or community-based living environment.
Specific Requirements
o Must be able to read, write, speak, and understand the English language.
o Must have a valid driver’s license.
o Must possess the ability to make independent decisions when circumstances warrant such action.
o Must be of general good health and demonstrate emotional stability.
o Possess good communication skills, both oral and written.
o Knowledge of the principles, methods, and standards of professional care coordination.
o Must possess the ability to deal tactfully with personnel, residents, family members, government personnel and the public.
o Must have the required experience and skills necessary to perform all essential job functions.
o Must have the ability to maintain a positive relationship with clients, their families, support personnel, etc., to assure the client’s’ needs are continually met.
o Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality service and maintaining a sound operation.
o Must have patience, tact, cheerful disposition and enthusiasm, as well as must be willing to handle clients and staff based on whatever maturity level at which they are currently functioning.
o Must possess the ability to seek out new methods and be willing to incorporate them into existing practices.
o Must be knowledgeable of computer systems, system applications, and other office equipment.
o Must not pose a direct threat to the health or safety of other individuals in the workplace.
o Must be able to perform all duties as assigned.
Hours : Full-Time Status, 32 hours a week, 64 hours per pay period.
