Position Overview
The Nurse Case Manager will oversee coordination of Healthy Start (HS) Program participants’ whole-person care, including assessments, care planning, team coordination, monitoring, and care transitions. This role is ideal for a compassionate, detail-oriented nurse who thrives in a collaborative environment focused on improving maternal health outcomes.
Key Responsibilities
- Lead the coordination of participants’ medical, behavioral, and social care needs.
- Partner with Community Health Worker Supervisors to facilitate non-clinical interventions, targeted education, and social support.
- Conduct clinical assessments using evidence-based tools under the guidance of program leadership.
- Develop and maintain participant-centered care plans.
- Coordinate and monitor implementation of care plans and triage changes in patient status.
- Facilitate transitions of care between home, hospital, and other healthcare settings.
- Lead daily huddles and weekly case conferences to review participant needs and outcomes.
- Conduct reassessments, update care plans, and manage participant discharges per program guidelines.
- Complete home, virtual, or telephonic visits as directed.
- Coordinate participant educational materials in collaboration with program leaders.
- Report significant medical changes and maintain accurate documentation.
Required Qualifications
Current unrestricted New York State Registered Nurse (RN) license (multi-state license preferred).Valid driver’s license , auto insurance, and reliable transportation.Current CPR / BLS certification.At least 3 years of patient care management experience , ideally in an obstetric or maternal health setting.Strong experience with electronic medical records (EMR) , health information systems, and documentation.Excellent written and verbal communication skills; proven success leading interdisciplinary care teams.Ability to work effectively in a diverse, fast-paced environment.Benefits

This position involves both field and office-based work with frequent travel for home visits, hospital coordination, and community partnerships. In-office work occurs in a traditional setting with conditioned air, artificial lighting, and open workspaces. Occasional lifting (up to 30 lbs.) and extended sitting may be required.
