Overview

Under the direction of the Director of Nursing, the Licensed Practical Nurse (LPN) is responsible for providing quality care to residents of the assigned neighborhood. The LPN functions as a professional caregiver who is accountable for planning, implementing and evaluating all aspects of resident care, including physical, psychosocial and environmental needs.

Essential Functions

• Documents according to nursing practice standards.
• Administers medications accurately and in a timely fashion and provides treatments to residents according to physician orders.
• Provides physical comfort and a safe environment.
• Accurately carries out physician / provider orders in a timely fashion.
• Initiates, implements, and evaluates care plans / MDS on an ongoing basis.
• Accurately completes admission and discharge procedures.
• Coordinates staff and resident care appropriately to meet resident needs.
• Follows all policies and procedures, annual mandatory in-services, required meetings, and consistently meets other conditions of employment.
• Performs other duties as assigned.

Per Diem and Flex Schedules Available

1st : 7 : 00am-3 : 30pm

2nd : 3 : 00pm - 11 : 30pm

3rd : 11 : 00pm - 7 : 30am

Pay Rate

Per Diem

$32.00 p / h + shift differentials ($2.00 p / h weekday evenings, nights & weekend days, $3.00 p / h weekend evenings and weekend nights)

Flex Per Diem

$37.00 p / h (must have 3 years of skilled nursing experience) + shift differentials ($2.00 p / h weekday evenings, nights & weekend days, $3.00 p / h weekend evenings and weekend nights)
