The Social Worker Designee provides support and advocacy for residents and their families within a healthcare or long-term care setting. This role ensures compliance with state and federal regulations, promotes resident rights, and assists in addressing psychosocial needs to enhance quality of life.
Key Responsibilities :
- Serve as a liaison between residents, families, and the care team to address concerns and ensure satisfaction.
- Assist with admissions, transfers, and discharges, ensuring smooth transitions and proper documentation.
- Support residents in coping with emotional, social, and environmental challenges.
- Maintain accurate records of resident interactions and care plans in compliance with regulatory standards.
- Educate residents and families on available resources, rights, and care options.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Participate in care conferences and advocate for resident needs.
Qualifications :
High school diploma or equivalent required; additional coursework in social services or healthcare preferred.Previous experience in a healthcare or long-term care setting strongly desired.Strong communication, problem-solving, and organizational skills.Ability to work collaboratively with diverse teams and maintain confidentiality.Knowledge of state and federal regulations related to resident care and rights.Preferred Skills :
