Home Health Care Coordinator

A company is looking for a Home Health Referral Care Coordinator.

Key Responsibilities

Provide staffing for home health and IV therapy referrals and assist with intake as needed

Ensure accuracy of demographic data and obtain necessary documentation to staff cases

Address inquiries and maintain positive relations with referral sources and vendors

Required Qualifications

Some college education and / or medical background; bilingual preferred

Experience with Work Comp insurance, referrals, and authorizations preferred

Knowledge of medical terminology and moderate PC skills, particularly in MS Office

Able to work cohesively with co-workers and external customers with minimal supervision

Knowledge of injured workers' rights and commitment to confidentiality
