Summary :
As a Patient Navigator / Health Educator, the associate will assess the education needs of workplace wellness partners. Design programs to address those needs and meet workers' varied schedules, with specific implementation plans, evaluation, and achievement of goals and measures. Align associate educational programs with the overall goals of the hospital and workplace wellness partners. Associates will also provide Navigation to educate and provide outreach to communities and high-risk populations. This individual provides support and advocates for those in the community who need extra help. This individual does not provide patient care and does not extend or substitute for the more specialized services of a doctor, nurse, or social worker. The Patient Navigator / Health Educator will assist patients in better utilizing the healthcare resources available and seeking care in the appropriate setting. The primary source of navigation will occur through telephone calls, but can include face-to-face encounters. The Educator / Navigator will be available for community events and hospital visits based on individual patient needs.
Responsibilities :
- Serves as a role model in support of healthy living. Coordinates and conducts comprehensive self-management health education programs and individual instruction, including group classes and health fairs.
- Assesses associate education needs and utilizes this data to plan and implement educational programs and services.
- Acts as liaison between hospital and wellness partners.
- Conducts periodic associate outcome evaluation. Maintains, prepares, and summarizes program evaluations, materials, and assessments, and revises programs in response to need, organizational change, and technology.
- Conducts individual and group education programs. Makes referrals to resources as needed. Maintains files, records, and other information in a professional manner following policy and procedures regarding confidentiality and HIPAA requirements.
- Compiles technical and statistical data for reports and record keeping. Attend required meetings and participate in committees as requested. Participates in professional development activities and maintains professional affiliations.
- Promotes and encourages teamwork by setting high performance standards for self, displaying a willingness to pitch in, being flexible, considerate, and fair. Maintains a positive attitude.
- Uses oral and written documentation skills to effectively convey ideas in a clear, positive manner that is consistent with the CHRISTUS mission, values, and standards of behavior. Follows all Policies and Procedures of CHRISTUS Health.
- Performs other health education / promotions as assigned. Knowledgeable in HIPAA regulations.
Requirements : Education / Skills :
Competency acquired through degree in Health Education, health promotions, or other clinical field or Community Health. Certified or working toward certification as health educator or wellness coach preferred.Experience :
Teaching, coaching, listening, communicating, creative and adept writing skills required. Must be comfortable with public speaking and have the ability to work with both individuals and large groups. Culturally sensitive and open to working with people of varied backgrounds. Position requires this person to be organized, self-directed, customer-focused, independent worker, and able to multi-task in a busy environmentLicenses, Registrations, or Certifications :
In accordance with the CHRISTUS Health License, Certification and Registration Verification Policy, all Associates are required to obtain the required certifications for their respective positions within the designated time frame.Work Schedule : 5 Days - 8 Hours
