Pay Rate : $1626.75 - $1782.50

Required Qualifications

Licenses : PT-WY

Certifications : BLS-AHA(Copy Needed)

SSN Required

DOB Required

Work History : Should not be more than 90 days of gap in last 1 year

License and SNF experience are required.
