  • Conduct thorough patient assessments and recognize changes in status to prioritize care based on clinical data.\n
  • Collaborate with patients, families, and the interdisciplinary healthcare team to create individualized care plans.\n
  • Continuously evaluate and update patient care plans based on ongoing assessments.\n
  • Respect and incorporate the cultural, spiritual, and belief systems of patients and families into care planning.\n
  • Coordinate discharge planning to ensure continuity of care post-discharge.\n
  • Deliver direct care aligned with established nursing protocols and institutional policies.\n
  • Delegate tasks efficiently, ensuring team members fulfill their responsibilities.\n
  • Monitor and ensure the quality of care provided to assigned patients.\n
  • Utilize resources smartly to enhance patient care while maintaining cost containment.\n
  • Maintain a safe environment, rigorously following established safety protocols.\n
  • Compile performance data to assess care quality and effectiveness.\n
  • Participate in performance improvement evaluations to enhance nursing practices.\n
  • Identify patient and family educational needs, providing tailored guidance and support.\n
  • Communicate effectively with the healthcare team about patient education requirements.\n
  • Maintain professional licensure and engage in continuous professional development.\n
  • Advocate for the patient's rights, coordinating care with all involved healthcare providers.\n
  • Perform additional duties as assigned to support patient care.\n\nSupervisory / Management Responsibility\n
  • This position does not involve management responsibilities; reports to supervisory or management personnel.\n\nMinimum Requirements\n
  • Education : Associate degree in Nursing (Bachelor's degree preferred).\n
  • Experience : At least one (1) year of experience as a registered nurse.\n\nIn Lieu Of\n
  • Successful completion of the Prisma Health Nurse Residency Program or a comparable residency may substitute for traditional experience requirements.\n
  • For internal candidates, one year of LPN experience in the same service line is acceptable.\n\nRequired Certifications, Registrations, Licenses\n
