Collaborate with healthcare providers to address patient questions, issues, and requests promptly. Schedule and coordinate patient appointments, consultations, and follow-up visits. Facilitate communication between healthcare providers, specialists, and other relevant staff to ensure comprehensive and integrated care. Provide patients with information about their diagnoses, treatment plans, and available resources. Educate patients on self-care practices and empower them to actively participate in their healthcare. Maintain accurate and up-to-date patient records, ensuring compliance with privacy regulations. Handle administrative tasks, such as billing inquiries, medical records requests, and documentation. Foster effective communication and collaboration among healthcare team members to optimize patient care. Serve as a point of contact for patients, addressing inquiries and providing information in a clear and empathetic manner. Participate in quality improvement initiatives to enhance the efficiency and effectiveness of patient care processes. Provide feedback on areas for improvement and contribute to the development of best practices. High school diploma or equivalent; additional education or training in healthcare administration is a plus. Experience being a liaison with insurance companies and patients Previous experience as a patient care coordinator or in a similar role is preferred. Strong interpersonal and customer service skills. Excellent communication skills, both in person and over the phone.
