What You Should Know About the RN Admission Care Liaison Role :

• The Admission Care Liaison is a DRIVING position supporting Ohio's Hospice South Region which cover the greater Miami Valley and surrounding areas
• Schedule hours are day / evening shift 11am-9 : 30pm with weekend rotations and 1-2 holidays per year
• We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed

The RN Admission Care Liaison Essential Duties Are :

Ohio’s Hospice patients will receive quality nursing care and coordination of hospice services, when the RN / LPN Admissions Care Liaison admits the patient into the hospice program efficiently and provides the patient and family with the appropriate resources needed at the end of life. The RN / LPN Admissions Care Liaison will maintain a conversion rate of 75% and skillful use of the electronic medical record by accurate documentation at the point of care.

RN Admissions Care Liaison Qualifications :

RN's - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred. Active Registered Nurse license in the State of Ohio without any board actions. Two years acute care nursing experience preferred.
