About The Position

Job Title : Physical Therapy Assistant

Profession : Rehabilitation Therapy

Duration : 13 weeks

Shift : Monday to Friday, Days

Hours per Shift : 32 guaranteed hours

Experience : Minimum years of experience not specified; New graduates are welcome.

License : State license verification required.

Certifications : CPR certification preferred.

Must-Have :

• Completed prescreen form attached upon submission.
• Resume attached.
• License verification attached.
• Three references provided.
• County criminal background check (7 years for all counties lived and worked).
• Federal criminal background check (7 years or more).
• Multi-state criminal search (7 years or more).
• Nationwide sex offender search (7 years or more).
• HHS / OIG search required.
• GSA search required.
• Social Security number trace completed.
• 1 Step PPD or Quantiferon or proof of positive PPD and chest X-ray.
• SAM search completed.
• DOJC (reran every 4 years with BID form).

Description :

Make sure the prescreen is filled out and attached upon submitting the candidate.

Resume and license verification also need to be attached.

If these items are not completed and attached, the candidate will be declined.

SNF experience preferred.

Covid test must be done 72 hours prior to the start date, with results issued 48 hours prior to the start date.
