Position Overview

Seeking a compassionate LPN to join a Primary Care / Community Health team providing comprehensive patient-centered care in an outpatient setting. The ideal candidate will assist in routine assessments, medication administration, and care coordination while ensuring a positive patient experience.

Job Details

  • Facility Type : Primary Care / Community Health Center
  • Unit : Outpatient / Primary Care (Pending Assignment)
  • Shift Type : Days, Monday–Friday
  • Guaranteed Hours : 40 per week
  • Float Requirement : TBD
  • Weekends : None required

    • Experience & Requirements

  • Experience : Prior Primary Care, Outpatient Clinic, or Community Health experience preferred
  • Certifications :

    • BLS (Required)

  • Active LPN License (North Carolina or Compact)
  • Travel Experience : Not required
  • EMR Experience : Epic or comparable system preferred

    • Core Responsibilities

  • Support providers in patient care, including vital signs, medication administration, and specimen collection
  • Assist with health assessments, wound care, and chronic disease monitoring
  • Maintain accurate and timely documentation in EMR
  • Promote patient education and preventive health practices
