About The Position

Responsibilities

• Conduct initial evaluations and ongoing assessments of residents' physical abilities and rehabilitation potential.
• Develop and implement individualized treatment plans to improve movement, function, and pain management.
• Provide therapeutic exercises, gait training, balance activities, and mobility interventions tailored to resident needs.
• Collaborate closely with an interdisciplinary care team-including OTs, SLPs, nurses, and physicians-to coordinate patient care and maximize outcomes.
• Document evaluations, daily progress notes, and discharge summaries in accordance with facility and state requirements.
• Educate residents, families, and caregivers on home exercise programs and techniques for continued improvement.
• Maintain compliance with all applicable federal, state, and facility regulations.

Qualifications Current, active Oregon Physical Therapist (PT) license required. Graduate of an accredited Physical Therapy program. Experience in a skilled nursing facility (SNF) or similar rehabilitation setting preferred. Strong communication, time management, and problem-solving skills. Compassionate and professional approach to patient care.
