Shift : MondayFriday days with potential weekend rotation and on call

Job Description :

Sunbelt Staffing is seeking a skilled Home Health RN for a travel assignment in Jefferson, IN. The nurse will provide in-home patient care, complete OASIS documentation, and coordinate with the care team. This role requires independence, strong clinical skills, and proficiency with Homecare Homebase.

Key Responsibilities :

Complete patient assessments and create individualized care plans

Perform accurate OASIS documentation in Homecare Homebase

Provide wound care, IV therapy, and skilled nursing visits

Educate patients and families on treatment and care management

Collaborate with physicians and the interdisciplinary team for optimal outcomes

Qualifications :

Active Indiana RN license

Minimum 2 years of nursing experience, with at least 1 year in home health

OASIS and Homecare Homebase experience required

Current BLS certification

Valid drivers license, reliable transportation, and insurance

Strong clinical judgment and communication skills

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
