Job Title : Resource Nurse (Hospice) — Admissions, Case Management, Visit Nurse

Pay : $90,000 – $130,000 DOE

Benefits : Full Benefits Package

Schedule : Monday – Friday plus on-call and weekend requirements.

Location : Serving the Greater Twin Cities Metro Area (21-County Service Area)

What You’ll Do

  • Admissions : Complete hospice eligibility assessments, consents / education, med reconciliation, immediate symptom plan, and safe-start services.
  • Case Management (as assigned) : Own a patient panel—plan of care, symptom management, caregiver coaching, IDT coordination, timely documentation.
  • Visit Nurse / Crisis Support : Provide PRN, routine, and urgent visits; execute standing orders; escalate and collaborate with the MD / NP swiftly.
  • Interdisciplinary Collaboration : Partner with social work, chaplaincy, aides, and therapy to support patient and family goals.
  • Documentation & Compliance : Chart accurately in the EMR; follow Medicare CoPs and agency policies.
  • What You’ll Bring

  • Active Minnesota RN license.
  • Required hospice background from one of the following : prior hospice agency experience or high-acuity settings such as ICU, palliative care, or oncology .
  • Strength in admissions interviewing, symptom triage, and family education.
  • Organized, decisive, and comfortable driving across the service area (reliable vehicle / insurance).
  • Professional, compassionate communicator who thrives in a dynamic environment.
