Location : Erdenheim, PA 19038
Compensation : $26 – $32 per hour (1099 Independent Contractor, no benefits / taxes provided)
Position Type : PRN – Temporary, flexible shifts
Contract Duration : 5 weeks

Qualifications
The ideal candidate will possess the following :
Bachelor’s degree from an accredited college or university (required)
Minimum of one year of related experience
Knowledge of child development (preferred)
Valid driver’s license and clean driving record
Position Details
The Early Intervention Service Coordinator will :
Maintain a caseload of 50+ families with minimum monthly contact
Serve as a liaison with Bucks County MH / DP, Behavioral Health, Children & Youth, referral agencies, advocacy groups, and stakeholder service providers
Ensure program compliance with all internal, state, and county policies, regulations, and requirements
Complete intakes of Bucks County policies and procedures
Facilitate and participate in assessments, evaluations, and the development, implementation, and review of IFSPs based on program regulations and family needs
Assess child needs and family concerns
Gather and document pertinent child development information
Coordinate the implementation of intervention services
Educate families on outcomes and family-training, routines-based models of intervention
Assist families in identifying appropriate service providers and facilitate communication between providers and families
Participate in the Local Interagency Coordinating Council (LICC)
Maintain all mandated family contacts and case management records in compliance with Bucks County MH / DP and PA state policies
Refer clients to BCIU #22 for transition services when appropriate
Attend and participate in transition planning meetings with BCIU #22
Ensure documentation is professional, accurate, and submitted on time
Input, update, and maintain records in the HCSIS Pelican system
Coordinate scheduling of IFSP / team meetings with families, therapists, and agencies
Obtain medical necessity forms from physicians when required
Research and share appropriate community activities and resources with families
Attend all required internal / external meetings, including at least 24 hours of training per year
