A company is looking for an Outpatient Care Management RN AMS.

Key Responsibilities

Provide comprehensive protocol-based management of Warfarin for enrolled patients

Coordinate care for patients with chronic diseases and facilitate medication management

Create personalized care plans and promote effective partnerships among patients, families, and healthcare teams

Required Qualifications

Licensed Registered Professional Nurse (RN) in Illinois

College Diploma in Nursing

Experience in nursing

Knowledge of anticoagulation therapy protocols

Ability to conduct holistic health assessments
