About the Position

A Federally Qualified Health Center in Hammond, Louisiana is seeking an experienced Family Nurse Practitioner to join its team. Requirements :

3–5 years of experience as a Nurse Practitioner

Active Louisiana NP license

Experience with both adult and pediatric patients

Schedule :

Monday–Thursday : 8 : 00 a.m. – 5 : 00 p.m.

Friday : 8 : 00 a.m. – 2 : 00 p.m.

Saturday–Sunday : Closed

Clinical Focus Areas :

Chronic disease management (Diabetes, Cardiovascular Disease, Asthma, COPD)

Preventive health services (Immunizations, Cancer Screenings, Wellness Checks)

Integrated Behavioral Health support

Same-day appointments for acute illnesses

Pediatric primary care

Competitive Salary : $100,000 – $145,000 (commensurate with experience)
