About The Position

Job Title : Registered Nurse Case Manager

Profession : Nursing

Specialty : Case Management

Duration : 13 weeks

Shift : Monday to Friday

Hours per Shift : 8 hours (8 : 00 AM - 4 : 30 PM)

Experience : Minimum one year of experience as a clinical nurse in a hospital required.

License : Current nursing license required.

Certifications : BLS certification required.

Must-Have :

Certified home health care experience required.

OASIS experience required.

Experience with EPIC or Netsmart preferred.

Description :

Develop and revise treatment and discharge plans in collaboration with patients, families, physicians, and care delivery teams.

Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition within defined time frames.

Process orders and notify physicians of patient needs and changes in condition.

Provide an environment that promotes respect for patients' privacy, confidentiality, and property.

Initiate appropriate preventive and rehabilitative strategies.

Provide services requiring specialized nursing skills.

Consult with and educate patients, families, and team members regarding disease processes, self-care techniques, and preventive strategies.

Supervise LPNs to ensure patient health, safety, and compliance with care plans.

Direct and evaluate the quality of patient care services provided.

Maintain positive interactions with patients, physicians, referral sources, and others.

Support care management philosophy, including collaborative planning and case management for individual healthcare needs.

Complete all required clinical records for compliance with legislation and seamless service delivery.

Demonstrate knowledge of available community-based services.

Exhibit flexibility to visit patients based on need rather than geography.

Resolve problems at the point of contact.

Ensure proper authorization and confidentiality forms are signed by patients or responsible parties.

Participate in patient care conferences for patients assigned.

Monitor results and take corrective actions on quality indicators, including customer satisfaction.

Understand the impact of the RN function on various healthcare aspects, including patient readmission and cost management.

Respond to findings from Quality Management reviews and ensure resolution of potential issues.

Participate in peer review and quality improvement initiatives as assigned.

Maintain productivity requirements.

Ensure appropriate visit authorizations are in place and that documentation reflects medical necessity for home care.

Contribute to a professional environment that fosters growth, learning, trust, and mutual respect among employees.

Promote recognition and celebration of workforce diversity.

Engage in educational activities, including training and self-development as necessary.

Demonstrate commitment to professional growth and competency.

Perform other duties as requested.
